The Duke Department of Community & Family Medicine, in partnership with the de Beaumont Foundation and the CDC, is holding its second annual Practical Playbook National Meeting, “Improving Population Health: Collaborative Strategies That Work,” May 31 to June 2 at the Westin Washington, D.C., City Center.

The meeting will bring together leaders and stakeholders in population health to share strategies on improving population health, implementing best practices, and developing action plans.

Carrie Horwitch, MD, FACP, a member of ACP's Board of Regents, is a member of the National Advisory Committee of the Practical Playbook.
